Dependent Eligibility Verification

All benefit-eligible employees should have now received a letter in the mail notifying them that they must provide supporting documentation to continue covering any dependent(s) listed on their insurance. Below is additional information and what to expect if you either have dependents, or are listed as a dependent, on a district provided insurance policy (medical, dental, or health; this does not apply to life or supplemental policies). If you do not carry insurance for a spouse, domestic partner, or child(ren) AND you are not covered by district insurance policy, you do not need to review or complete any of the tasks below.

Review Requirement

The Oregon Educators Benefit Board (OEBB) requires Roseburg Schools to complete a Dependent Eligibility Verification review. Dependents include an employee’s spouse, domestic partner, and/or child(ren) enrolled in an employee health, vision, and/or dental plan. Our district is partnering with American Fidelity to provide employees with additional resources and assistance in completing this review. This review takes place every five years.

Employees who cover any dependent(s) are required to meet with an American Fidelity representative to provide documentation verifying their dependent(s)’ eligibility and to sign the Dependent Verification Review form. Employees’ personal documentation will be viewed but will not be retained by American Fidelity or the district. All information is confidential.

Meetings with American Fidelity representatives are scheduled 03/30/26-4/30/2026.

Employees can make an appointment by visiting the following site: https://enroll.americanfidelity.com/827826CFAmerican Fidelity Meeting Link. Only the employee who is the primary subscriber needs to make an appointment and present their supporting documentation; dependents are not required to be at the meeting.

This link will also be available on posters distributed throughout school and administrative buildings near labor laws posters and other common areas.

Failure to meet with an American Fidelity representative and provide proof of dependent eligibility by 4/30/2026 will result in a termination of coverage for dependent(s) effective 5/1/2026. Employees who do not have dependents covered by their medical, dental, and/or vision policies are welcome to still meet with American Fidelity to go over optional products and review current plans but are not required to do so.

Appointments will be held both in person and virtually from 8:00 a.m. to 4:00 p.m. The individual one-on-one appointments should not take more than 30 minutes. Employees may be offered American Fidelity products; however, there is no obligation to purchase.

Open Enrollment starts Aug. 15, 2025

Open Enrollment is the only time each year you can change your medical, dental, or vision benefits without a major life event.

Starting Aug. 15, go to the OEBB Enrollment Link and make your selections during Open Enrollment. This includes:
•    Making plan selections for insurance and supplemental benefits.
•    Adding or dropping a dependent.
•    Updating your personal information or beneficiaries.
•    Enrolling as a new hire.

Open Enrollment for Roseburg Schools occurs each year from August 15th to September 15th for benefits that begin on October 1stThe right benefits can help you stay financially stable. Open Enrollment ends Sept. 15 at 11:59 PM.

Note: coverage for dependents will end 5/1/26 if documentation is not provided to American Fidelity by 4/30/26.
